The Medicare Learning Network has issued an alert to remind all providers that practice address on file must be physical locations and not a PO Box or Lockbox address. Claims submitted with a PO Box as the provider address won’t be crossed over and your EOMB will reflect rejection code H25375. You may report a PO Box or Lockbox address as the “Pay-to Provider Address” but not the “Billing Provider Address”. If you’ve received these rejections from Medicare, call your FI, Carrier, DME MAC, or A/B MAC to confirm what street address information Medicare has on file for you.
